Unexpected Key changes

Why do my charts automatically key change and keep playing - even though I've set it to 3 times through?
It's like some kind of default setting ..
Also How do you create an ending ?

There is a button that looks like a graduation hat (in the player controls) and you can set it up there.
For the ending, what would you like exactly?

I'm finding it difficult to work out where all the dc al coda operations are - I can see the coda and funky s symbol...any tutes on this ? Thanks
