Thanks for this, and yes that structure is correct. In the second A1, the F# half dim becomes a D major, I was quite confused how to write this though.

Yeah, that form is correct. The 'D third time' refers to a change from the F# half dim to a D major on the second A1.

My earlier post was delayed. Sorry for the confusion. How best to chart a tune is often affected by the one-page constraint.
Currently, the player can only handle one page at a time.
