Not sure if there's a neater way of writing it out. Structure is A B1 A B2 A C B2 A(an adlib repeat)
I had difficulties figuring out how to play back parts in the correct order, but it works.

DC/DS al Fine gives you a second chance to insert something between repeats. You can go DC/DS al Coda from there.
I tried avoiding the coda and having to respell out the final B2 A, but couldn't get DC/DS al X end. to work in the same way.

DC/DS al Coda includes the coda as part of the form every time it repeats.
