The C add9 is the basic C shape with pinky on the d note second string. Your pinky holds that note through the walk down and for both the D/A and A7 sus. It's really handy because you can go from G to G minor, because there's no upper third note. That D note on the 2nd string is also the highest note in Bb^7 and C11 chords. It makes the song, and it's the "telegraph note".
C11 is a straight bar across the third fret skipping the lowest string (I skip the highest string too). The F^7/A is the same shape as an a minor chord.

BopCat's version is very good. If I could just add my contribution, the C after the chorus which sounds better as a Cadd2 rather than a C11. Although at the intro and verse the C11 is correct.
