Take Five - Paul Desmond
This is the chart with the chords from the Real Book, along with an intro and a section for solos so it can be more akin to the recording. Be sure to have the repeat setting on an even number due to where I put the end.
With the repeat set to 2, you will get:
Intro (8 bars)
Head In (A-B-A, 24 Bars)
Solos (marked with a "V," 4 choruses of 16 bars each [or you can think of it as 8 choruses of 8 bars each, i.e. the A section 8 times])
Head Out (A-B-A, 24 Bars)

Yes. Mine are from the published sheet music.
I've stopped using any published music sheet (or fake books) about 20 years ago once I realized how inaccurate they were.
I've also realized that the best way to learn tunes is to transcribe as many versions as possible, often starting with the original version that was performed by the composer (when available). That gives a broader view of tune.
Sometimes some cover versions can be as good or even better than the original (but not always).
I always think a chord chart as a guide. When performing, I would play according to what I hear at the moment, never strictly following specific chord changes in a chart. But to really understand the relationship between the melody and the harmony, a chart should be as accurate as possible. It's also more helpful when playing with musicians that have never heard the tune before.

There are surely a few books out there that are accurate. Yours is probably one of them.
I have (actually "had") about 50 PDF fake books. There were errors in all of them!
This is one of the reason I'm using iReal pro: I can edit any chord, structure of any song. This is much easier (and faster) to do than with a paper book or a PDF music sheet. When I need to write melodies or tunes that iReal Pro can't handle, I just use Finale (or Sibelius)

lol Absolutely not. Have you ever heard take five the whole reason why Buck wrote it it’s in 5/4 is a disgrace. Not called take two. Called take five….. come in now!!!!
