That's a nice tune.
The sheet music I've found for it is slightly different (aside from not including the Intro):

I suggest a couple of things for your chart.
It copied down in C, but the default key is Db.
The time signature of 4/4 should be at the top of the tune.
Label the 9th bar the "A" section. Otherwise the whole tune repeats each time, including the Intro.
You have a couple of ending barlines before the actual ending.
The tempo is more Medium Up Swing (160) than Medium Swing (100).
