Form is AABA, which I did with DC al Fine. If there's a better way, I'm open to change.

Your chart is just fine, thank you.

Since each bar only has one chord, you could condense the chart using 8 2-space measures per systen (line) and write out the whole form top to bottom and still only use 8 lines.......but why?
