This author of the tune is a bit confusing, as one sheet attributes it to Art Pepper and one to Chet Baker.

The YouTube version given above, aside from being in F minor, doesn't quite match the changes or the melody in either sheet.

Here's the one I copied the changes from, attributed to Art Pepper:

The other sheet attributes the tune to Chet Baker, and has the same melody with slightly different changes.

(Edit: The composer name was corrected in the next post by Bob. It's Art Pepper.)

These are both in D minor, which is clearly the transposed trumpet part, as Chet and Art play it together in concert C minor:
