    As said before, label the head as Intro, then label solo choruses A or B, use D.S. to go back to head. use fine or coda with the D.S.

    Blowing changes sound like this to me: Cm11 6 bars - Bmaj9+11 6 bars - Bb7sus 4bars - C7sus 2 bars - F7sus 4 bars. Listening to various recordings of this tune, the C7sus is not really played very much, so I would probably keep playing the Bb7sus for 6 bars.

    The Bb-11 and C-11 chords should be Bb9sus and C9sus or Fm7/Bb and Gm7/C. Bb-11 inplies a Db and the C-11 implies an Eb in the chords. Neither of those notes are in the chords. Check the tune out on You Tube.
