It would be helpful if you included a time signature. Your credit for songwriter is incorrect (Eddie Harris only), and according to the Sher Music score I have, there are only four chords in this song (unless this is a different version other than Eddie Harris' you are transcribing).

Formatting is off a bit, but my chords sync with yours. If someone could combine your chords with CyrilleB's formatting, I think that would be ideal.
